Providing Pathways to Health
We are excited to share that Dignity Health – St. Mary Medical Center is continuing to support our Pathways to Health program at the Century Villages at Cabrillo (CVC), with a 2023 Community Health Improvements Grant of $97,000. In this third phase of Pathways to Health, we are increasing our focus on mental health assessment and interventions, access to healthcare and screening, and services focusing on healthy eating and lifestyles. We are also adding financial education to help residents make good financial decisions to retain their housing and thrive. Launched in 2017 with original funding from Dignity Health, the Pathways to Health program is grounded in the belief that wellness is more than the absence of illness or disease. Its unique strategy focuses on individualized wellness planning and implementation. Partnerships with healthcare providers and other organizations provide the resources each resident needs to stay on their personal path to wellness.
Details on Pathways to Housing Program in 2023
This year, in collaboration with Dignity Health – St. Mary Medical Center, CityHeART, Sowing Seeds of Change, CSU Dominguez Hills’ Masters in Occupational Therapy Program, and Farmers & Merchant Bank, CVC will offer an array of services that provide a continuum of care: direct services, community engagement activities, behavioral health interventions, support groups, health and wellness screenings, gardening and healthy cooking education, financial workshops and more to provide residents the support needed to remain housed and focus on their emotional, financial, and physical health.
In addition to case management services, Century’s Oasis Residential Services team will create several opportunities for residents to engage and be part of their local community through monthly Village Community Meetings, presentations and workshops, and CVC community-wide events throughout the year. CVC’s behavioral health specialist is available to the entire Villages community to provide short-term, goal-focused, and evidence-based interventions for Village residents and respond to situations that, without intervention, could lead to lease violations and the potential loss of housing, conflicts with other residents, or police involvement.
Dignity Health – St. Mary’s Medical Center will continue to have its mobile unit come onto the CVC campus monthly with a team of professionals to provide the following services to ensure our residents are monitoring their health and maintaining a connection to health professionals and address any concerning issues and/or receive treatment when needed: blood pressure screenings, BMI screenings, cholesterol screenings, glucose screenings, respiratory tests, and more.
CityHeART will provide access to health and wellness program, which includes support groups addressing substance use, yoga classes, art lessons, music therapy, and more. They will also be providing a weekly wellness clinic facilitated by CSULB nursing students.
Sowing Seeds of Change will bring the expertise of a certified ecological horticulturist to provide a series of gardening workshops and community engagement activities throughout the year promoting sustainable gardening, food sovereignty, and overall health and wellness. Cooking demonstrations will also take place, including family cook-offs and crop contests.
Our occupational therapy interns from CSUDH will work with residents in group and individual settings to increase independence and healthy living.
Farmers and Merchant Bank, Long Beach will provide workshops and trainings on financial education, banking, taxes, etc.
